Lacks leaves K1 and Q3, and gatherings Gg-Hh with "Tractatus de urinis"; gathering e damaged, slightly affecting text. Early manuscript notes in English, some being English names of plants or recipes; English names of plants added by hand in general table. Contemporary calf binding, blind tooled with floral motives, roses and fleur-de-lys stamps, repaired in parts and coated with glue; remains of 2 clasps catching on front cover (their larger side reproducing feathers); fragments of manuscript on vellum between gatherings a and b. Ownership inscriptions on title page: "Thomas cracokes", "Ri: Phillipps", "John Gaylor", "Ambrosius westburiensis Episcopus Aprill 27. 1666 ..."; early device with initials "T C" on verso of leaf Ff8v (in pencil, covered with black ink, as later inscription below colophon reading: "Printed at Mentz 1483"); the same device inscribed on upper edge in brown ink; inscription on leaf l3v "John shines wyll[ia]m nycole corbe"; inscription on t.p. verso: "d.
